To apply to become a CCF/R volunteer, please complete and bring to us the following forms and items. You will need to print out any forms and bring them to CCF/R Headquarters. All items will need to be complete. Click your browser back button to come back to this page.
- Criminal Background Check. You can obtain this through the State of Alaska Records and Identification. (907) 465-4343
- Driving Record. You can obtain this through the State of Alaska Division of Motor Vehicles. (907) 465-4361
